I desire to participate in  Events sponsored by Latin dreams Amsterdam Bachata Dancers, including inter alia   Kizombachata Saturday Socials (monthly) and the Latin dreams Amsterdam Bachata Congress (annually) (the “Latin dreams Amsterdam Bachata Events”) and, in consideration of being allowed to participate in the Latin dreams Amsterdam Bachata Events, I agree as follows:

• I acknowledge I must be an active participant in my own healthcare and that personal responsibility is imperative regarding the COVID-19 virus.
• I agree to follow all Latin dreams Amsterdam Bachata Dancers,  policies and procedures regarding COVID-19 while participating in the  Events. I understand that I may be removed from the  Events if I do not comply with Latin dreams Amsterdam Bachata Dancers, Inc. policies and procedures.
• I will not participate in any Latin dreams Amsterdam Bachata Event while I have any of the following symptoms: respiratory symptoms (such as dry cough or shortness of breath), a fever of 100.4 degrees or higher, sore throat, chills, body fatigue or aches, loss of taste or smell, headache, congestion or runny nose, nausea or vomiting, or diarrhea.I understand that COVID-19 is a highly contagious disease and I may contract COVID-19 even if I follow the safety precautions above.
• I also understand that, although Latin dreams Amsterdam Bachata Dancers,  will follow guidance from the  Netherlands and local authorities to reduce the spread of COVID-19, Latin dreams Amsterdam Bachata Dancers,  cannot completely eliminate the risk of individuals participating in the Latin dreams Amsterdam Bachata Events from contracting COVID-19. I agree to voluntarily assume all risks related to the COVID-19 virus. I am fully informed or otherwise aware of, and fully assume, all risks to person and property in connection with my participation in the Latin dreams Amsterdam Bachata Events, including but not limited to property damage and loss, bodily injuries, sickness, disease and death.
• I fully RELEASE, WAIVE AND DISCHARGE, and COVENANT NOT TO SUE, Latin dreams Amsterdam Bachata Dancers,  (including, but not limited to, its directors, advisors, volunteers, staff employees and representatives) from and for any and all demands, claims, actions, suits, damages, losses, liabilities, costs and expenses (including but not limited to court costs and attorneys’ fees), from any cause whatsoever (including but not limited to property damage and loss, bodily injuries, sickness, disease and death), directly or indirectly due to the COVID-19 virus, whether or not foreseeable or contributed to by the negligent acts or omissions of Latin dreams Amsterdam Bachata Dancers,  or others.
• I have read and fully understand this Latin dreams Amsterdam Bachata Dancers,  Covid-19 Virus Liability Waiver Agreement and intend that this Agreement be legally binding and enforceable upon me and my family, estate, heirs and legal representatives.
